When filling in the Medical Section of this form, you are providing information about your child's health (mental, physical, hauora) and family circumstances that we need to be aware of.

Specifically, please comment and provide details on the following as appropriate:

  • What is your child's immunisation status?

  • If your child requires medicine to be dispensed to them during school time, please request an authorisation form to complete for this (this will be sent to you to complete and return to the college).

  • 󠆺  Specialist Reports – e.g. Education Psychologist/Hospital Reports providing confirmation of any diagnosis 
  • 󠆺  Custody Documents / Court Orders (if applicable)
